I understand that under Article 26 of the Freedom of Information and Protection of Privacy Act, the Central Okanagan Public Schools is authorized to collect the information in this form to provide educational services for my child.

I authorize Central Okanagan Public Schools (SD No. 23) to request and collect my child’s student records from previous schools and to share the information I have provided with the schools that I have chosen as preferences.

I understand that the information I have provided in this form will be entered and stored in the BC Ministry of Education’s provincial student information system (MyEducationBC) and used for the purposes of delivering and tracking my child’s education in accordance with the requirements of the School Act.

In addition, I understand that the contact information that I have provided in this form will be used in condition of service applications such as SchoolMessenger and Microsoft 365 (learn more at https://www.sd23ltd.com/district-applications).
